Employee's hand fractured when caught in machine feed

Employee #1 was feeding newspaper lead into the folding and assembly line when there was a break in the moving paper line. He was using the normal speed mode to advance the broken newspaper section into the unit when his hand became caught in the machine and was broken. Employee #1 should have been using the jog control feature.
