Investigation Summary
At 7:20 a.m. on February 19, 1993, Employee #1 was on his way to a hot stack to pick up some stock with his forklift. As he began his turn, the forklift began to skid and he hit his brakes, which increased his skid. He was heading toward the rail car pit. Not wanting to go into the pit, he panicked and jumped toward the skid. The right front wheel slid off into the pit and he landed on his stomach facing north. His right leg was pinned beneath the forklift and his left leg was between a unit of panels and the forklift. The forklift was immediately raised with a hydraulic jack to relieve pressure and then a large forklift was used to raise the forklift to remove Employee #1. Emergency crews arrived and transported the employee to Oroville Hospital, where he was treated for a broken right kneecap along with major abrasions of the right leg and minor abrasions of the left leg. A water puddle on the concrete floor from a recent downpour allegedly caused the forklift to skid.
