Investigation Summary
At 9:00 p.m. on December 13, 1995, Employee #1, of Sierra Pacific Industries, was in the basement of an old sawmill attempting to clear a jam in the chain conveyor. He had shut off the main conveyor and climbed onto it to clear the jam. This maneuver, which he did several times each shift, involved using a steel bar to pull sawdust and pieces of board off the slushier chain conveyor while it was running. He was reaching for a piece of board when his sweatshirt became caught and he was pulled into the sprocket nip point. Employee #1 sustained a dislocated right shoulder, right elbow, and right wrist (including a chip fracture); almost complete amputation of his right ear; lacerations to his forehead, nose, right knee, left ear, right arm and hand, and the top of his head. He also suffered from numbness in his left arm. The accident resulted from working near the unguarded, running chain conveyor sprockets.
