Employee's fingertips amputated when caught against winch

At 2:00 p.m. on October 11, 1995, Employee #1, a choker setter for Bigelow Land and Timber, was working with a tractor operator at a logging operation. The tractor was a John Deere 440B rubber-tired skidder with a rear-mounted winch. The 5/8 in. diameter winch cable had a small bull hook and clevis; the chokers were 1/2 in. steel cable. Employee #1 did not get the tractor operator's attention prior to reaching in to set the chokers on the bull hook. The driver, unaware that Employee #1 had grabbed the bull line, began to pull in the slack on the winch line. Employee #1's fingers became pinched between the bull line and the horizontal winch rollers. The tips of his left index and middle fingers were amputated.
