EMPLOYEE CAUGHT A FINGER IN NIP POINT OF A MACHINE

At 3:20 a.m. on June 8, 1994, Employee #1 was threading plastic film in a bagging machine, Model Number 718504100, for Most Packing Machine when the film jammed. Employee #1 opened the guard and jogged the machine. When she reached in and pulled the film, her finger was caught in the nip point of the rotating end seal jaws. Employee #1 had the tip of her left middle finger amputated but was not hospitalized. Employee #1 was using a jog switch near the nip point involved in the accident.
