EMPLOYEE INJURED IN FALL FROM LADDER

At approximately 9:40 p.m. on April 12, 1994, Employee #1 was working alone repairing a conveyor bearing. When he stepped on the fourth rung of an 8-ft Louisville Model FS1008 Type 1-A fiberglass stepladder, the step failed and he fell to the floor. Employee #1 fractured his right leg requiring surgery to install pins. The fourth step of the ladder had a tear in the vertical front section of the aluminum step with a 1-in. drop at the front edge. The work area was very clean where dried fruit is processed. The work surface had recently been replaced with a new non-skid concrete surface.
