Investigation Summary

Investigation Nr: 170360325
Event: 06/08/1995
Employee burned trying to put out fire at residence

At 10:00 a.m. on June 8, 1995, Employee #1 and a coworker were doing road maintenance when they saw smoke in an area to the south of them where they knew an elderly couple lived. Employee #1 and the coworker arrived at the site, parked along the county road, and walked back to the elderly couple's house where they saw a small county tanker truck and a single fire fighter. A small shed was engulfed in flames and the grass was on fire. The fire fighter yelled at them to grab hoses and help. The employees and the fire fighter were hooking up a hose downhill of the shed when a 150 lb LPG tank exploded. Employee #1 was seriously burned on his arms and lower back; the coworker sustained a minor burn on his right elbow. The elderly male resident died of the injuries he sustained. The employees did not know that a propane tank was stored in the shed.
