Employee injured when catches fingers in screw conveyor tube

On April 5, 1995, Employee #1 was working on the first floor of a newly installed and fabricated "line plant" facility which was not yet fully operational. Employee #1 was testing the quality of the grinding process by removing a sample of material from the completely covered 12 inch diameter screw auger conveyor. He first removed two nuts that held a plate over an inspection opening measuring 6 inches by 6 inches. He then caught the material in his hand as it dropped through the opening. For some reason he placed his fingers within the conveyor tube. The flights of the screw conveyor severely damaged two fingers; the index finger on the left hand was later amputated at the knuckle.
