Investigation Summary
At 7:30 a.m. on April 19, 1995, Employee #1 and a coworker were using a Hyster Forklift, model #50 XL, to move some roofing materials. The truck's front tires are pneumatic, while the rear wheels are solid. The forklift has a maximum limit of 4,600 lb. The coworker, who was operating the forklift, had picked up the roofing materials and was returning to a customer's truck with Employee #1 riding on the pallet of the small load. Employee #1 lost his balance on a sudden turn and fell off. He was run over by the right front wheel, but the forklift was stopped before the rear wheel reached him. Employee #1 suffered a fractured pelvis, and fibular and soft tissue abrasions to the knee, hands, and body. The accident was caused by the employee riding on the load of the forklift.
