Investigation Summary
At 5:30 p.m. on November 19, 1994, Employee #1, of Quaker Oats, was operating a bank of machines and attempting to clean rice cake machine #37. As he reached into the machine to wipe away some rice residue, the machine cycled, catching his right hand in the nip point. The top of his right second, third, and fourth fingers sustained 2nd- and 3rd-degree burns before he could stop the machine. Employee #1 did not lock out the machine before reaching into the danger zone.
