Employee's fingertip amputated by shaper

At approximately 12:00 noon on March 14, 1992, Employee #1, of Cabin Creek Woodwork, removed the automatic feeder from a shaper to run an edge on a 12 in. wide by 40 in. long board. According to Employee #1, the job could not be properly done with the automatic feed in place, so he held the board with both hands while he fed the board into the shaper. Employee #1's left hand was past the shaper head and his right hand was pushing the board against the shaper head. His thumb was on the edge of the board and his fingers were on top of the board. His thumb slipped off the edge of the board and the momentum of his pushing carried his hand into the shaper head. Employee #1's right ring fingertip was amputated just below the fingernail. Employee #1 had been working for this employer for more than 6 years and operating the shaper was part of his job. Employee #1 also trains other employees how to operate the shaper. The employer is being cited for a repeat regulatory violation of T8CCR 342(a) and a repeat general violation of T8CCR 3203(a). Note: 4318.1 is a class B guarding requirement.
