Employee's eye injured by piece of flying wood

At approximately 12:30 p.m. on September 25, 1991, Employee #1 was cutting a piece of lumber with a circular saw when the wind, which was blowing into the building through big double doors, blew a piece of wood into his eye. Employee #1 removed the piece of wood from his eye, but some residue remained and caused considerable pain. He was taken to Portola, where he was treated in a doctor's office and released. No further treatment was required and Employee #1 returned to work the next day. This was not an accident that required reporting to CAL/OSHA. No safety violations are being issued; however, a general violation is being issued because the employer did not have an injury and illness prevention program that met industry requirements.
