Employee's fingers amputated by using a radial arm saw

At approximately 1:40 p.m. on February 8, 1992, Employee #1, of Meeks Building Center, was cutting a 1 by 12 to length for a customer using a DeWalt radial arm saw. He pushed the board to a mark on the table indicating length, then pushed the board to the back of the table. As he was pushing the wood, his left hand slipped and went into the moving saw blade that was at the back of the table. He cut his middle and index fingers. The middle finger had to be amputated at the first joint. Employee #1 had received training on the saw as well as general safety instructions. He also attended woodworking classes during high school, including building of complete pieces of furniture, and had extensive training on all types of saws. The saw was guarded to standard and no violations were written pertaining to the accident. One regulatory and one general citation were issued for other violations.
