Employee's foot broken by falling steel

At approximately 8:43 a.m. on August 26, 1992, Employee #1 was assisting with a crane lift of some large pieces of steel. Employee #1 rigged the load unevenly and it came loose as the crane picked it up. Employee #1 was standing too close to the load and, when it fell, a portion struck and broke his foot. Employee #1 and the crane operator were both long time employees, experienced and trained in their jobs. The equipment being used was in good condition and appropriate for the job. The employer had a good injury and illness prevention program including self inspections, safety meetings, and training. No citations are being issued relating to the accident. A regulatory citation is being issued for failure to report serious injury, T8CCR 342(a).
