Employee's heel injured in fall from ladder
On December 18, 1996, Employee #1 was using a 69 in. stepladder to retrieve bean bags from a metal storage rack that was 105 in. above the concrete floor of a warehouse. She was reaching out when the stepladder moved. Employee #1 fell and shattered her left heel.
