Employee's arm broken when caught in conveyor rollers

At approximately 3:15 p.m. on December 11, 1995, Employee #1, working for a manufacturer of pressed boards, was cleaning the underside of a 102 in. wide rubber dip take-up conveyor belt (green belt pulley line #1) from the north bottom location near the nose bar. The rag he was using got caught between the upstream 9 in. diameter roller and the middle 12 in. diameter roller, 51 in. and 55 1/2 in., respectively, above the concrete floor, and pulled his arm between them. Employee #1 fractured his right arm at the radius and ulna. The conveyor belt and rollers were unguarded and he did not lock out the conveyor before cleaning.
