Employee's thumb amputated by blade on table saw

At approximately 6:00 p.m. on April 13, 1992, Employee #1, of Dreamwood Cabinets, was cutting scrap lumber into smaller pieces so they would fit into a garbage can during cleanup. Employee #1 had worked a 12-hour shift and was tired. He was attempting to rip approximately 3 7/8 in. off a 20 in. long by 8 in. wide piece of lumber. When he pushed the piece of lumber past the saw blade with the push stick, the lumber apparently grabbed and pulled his hand into the blade. Employee #1's right thumb was amputated. The thumb was reattached at California Pacific Medical Center in San Francisco. The reattachment appeared to be successful. Employee #1 is experienced, and has been building cabinets for 2 1/2 years. The table saw he was using was not guarded as required. The employer is being cited a general violation of T8CCR 3203(a)(2) and a serious violation of T8CCR 4300(a).
