EMPLOYEE IS STRUCK BY INADEQUATELY SECURED OBJECT

AT APPROXIMATELY 11:30 A.M. ON FEBRUARY 11, 1992, EMPLOYEE #1 WAS INJURED WHEN HE WAS STRUCK AND PINNED BY A 2,634-POUND GOLD ORE CONVEYOR TRANSFER BOX THAT FELL FROM THE TAIL SECTION OF A RADIAL STACKING CONVEYOR BEING ERECTED BY ANOTHER COMPANY. THE CREWS WERE IN THE PROCESS OF SETTING THE CONVEYOR INTO THE TOWER SADDLES. IT WAS LATER DETERMINED THAT ELEVATING THE TOWER END OF THE CONVEYOR ALLOWED A FLANGE OF THE TRANSFER BOX TO CATCH AND HANG UP ON THE GEAR HOUSING OF A CHAIN HANGING FREE BESIDE THE TRANSFER BOX. UNAWARE OF THIS SITUATION, THE CREWS WERE UNABLE TO ALIGN THE ASSEMBLY AND INSTALL THE BOLTS AND PINS. EMPLOYEE #1 WENT TO RETRIEVE THE CHAINS NEEDED FOR ALIGNMENT. EMPLOYEE #1 WAS STANDING DIRECTLY BESIDE THE TRANSFER BOX WHEN HE UNKNOWINGLY TWISTED THE GEAR BOX CASE OF THE CHAIN HOIST OUT FROM UNDER THE LIP OF THE TRANSFER BOX ON WHICH IT WAS CAUGHT. IMMEDIATELY AND WITHOUT WARNING, THE TRANSFER BOX BEGAN TO SLIDE OFF OF THE CONVEYOR. EMPLOYEE #1 JUMPED TO THE GROUND TO GET CLEAR OF THE FALLING BOX AND BECAME MIRED IN THE SURROUNDING MUD, WHERE HE WAS SERIOUSLY INJURED WHEN STRUCK BY THE TRANSFER BOX.
