Investigation Summary
At approximately 8:40 a.m. on January 9, 1995, Employee #1, of PPS Packaging, was removing excelsior from the draw roller of a cooler pad machine, when his right arm became caught in the unguarded moving roller. He sustained a serious injury to his right arm and was taken to Valley Medical Center, Fresno, CA, where he was admitted for treatment.
