EMPLOYEE SUFFERS FINGER AMPUTATION

At approximately 9:00 a.m. on May 17, 1998, Employee #1 was working at a box and packaging manufacturing company stacking the finished large cardboard sheets at the end of the production line. In this process, the cardboard sheets are fed from a table into a machine that coats the cardboard sheets with a glue-like substance. The machine consists of three 64-in.-long rollers. The glue-like substance is applied from above the line. Earlier that morning the employees had to wipe glue off the rollers due to a problem with the glue dispenser. This was done with the machine running. The employees continued to work until break time when they were still having problems with the glue dispenser. Employee #1 observed two employees wiping off the rollers again and decided to help them as she had done earlier that morning. The table that was usually in front of the rollers had been moved away. Employee #1 grabbed a rag and was wiping the rollers when the rag became caught in the machine and pulled her hand into the rollers. This resulted in a complete amputation of the index and little finger on the left hand. Employee #1 was hospitalized.
