Employee falls from unsecured ladder

Employee #1 was using a 16-foot extension ladder to climb a roof so that he could show his supervisor some problems with work that had been done earlier. Upon reaching the roof, he started to swing his right leg onto it when he and the ladder fell. Employee #1 sustained a fractured left elbow in the 10-foot free-fall to a concrete surface. The ladder that Employee #1 used was not secured to prevent displacement.
