Investigation Summary
At approximately 5:15 a.m. on December 31, 1992, Employee #1 of Kovacevich Farms, Arvin, CA, was working on rig #12 during water well drilling operations. He was on the floor, and had just opened an air valve to blow drill cuttings when the hose to the kelly came loose and fell approximately 10 feet onto him. He was taken to Mercy Hospital, Bakersfield, for treatment of four broken ribs.
