Investigation Summary
At about 1:30 p.m. on December 2, 1992, Employee #1 of Chuck Haddad Mitsubishi was inflating a tire when it ruptured. The employee had installed a 16 inch tire on a 16 1/2 inch rim. He overinflated the tire, causing the tire to rupture. The employee sustained contusions and a shoulder injury, for which he was treated and released.
