At approximately 8:30 a.m. on March 25, 1992, Employee #1, of Grimmway Farms, was using an hydraulic jack to elevate a forklift truck. As Employee #1 placed a safety block underneath, the fork truck descended, crushing his left thumb. Employee #1 was hospitalized and his left thumb was amputated.
