Investigation Summary
At approximately 8:00 a.m. on September 27, 1991, Employee #1 was working on workover rig #618 at the Arco Oil Lease in Fellows, CA. As he moved to stay clear of a falling circulating hose with a pipe swivel, his forehead struck against the tongs. He suffered a laceration and was taken to Memorial Hospital, Bakersfield, where he was treated and released the same day.
