Employee injured in fall from roof

At approximately 10:30 a.m. on February 28, 1991, Employee #1 was installing a sheet metal roof at a height of about 20 ft. He placed a panel of roofing on three support beams and the roof valley. He was preparing to put in a screw when the panel slipped off the valley and off one of the steel supports. Employee #1, who had his foot on the panel, fell to the floor. Both his ankles and his left heel were fractured, and he suffered a dislocated shoulder. His left eye socket was also fractured when his face struck a sprinkler pipe during the fall.
