Investigation Summary
At 12:50 p.m. on July 23,1997, Employee #1 sustained an amputation of the tip of his left ring finger. This incident occurred at the Los Angeles City Department of Water and Power facility located at 201 South Webster Street in Independence, California. Employee #1 was a mechanic and crane inspector with 12 years experience. At the time of the incident, he was rewinding cable on the drum of a 14-ton National Crane. He was standing atop the cab guiding the cable onto the drum with a hammer, as the crane operator ran the controls from the control station on the left side of the crane in back of the cab. While Employee #1 was guiding the cable onto the drum, he rested his hand on top of the boom and inadvertently placed his finger in a 1.75-inch-diameter wear-pad access hole in the outer boom section. When the inner boom section retracted, as it passed the access hole, it sheared off the tip of Employee #1's finger.
