Investigation Summary
AT APPROXIMATELY 3:55 PM ON DECEMBER 26, 1990, EMPLOYEE #1 SUSTAINED SERIOUS INJURIES WHEN THE TRENCH IN WHICH HE WAS WORKING COLLAPSED. THE EMPLOYEE WAS WORKING IN A 6 1/2-FOOT SECTION OF A 40-FOOT TRENCH. FIVE FEET OF THE TRENCH WERE 78 INCHES DEEP AND THE REMAINDER WAS 54 INCHES DEEP. EMPLOYEE #1 AND A CO-WORKER WERE DIGGING UNDER 3 PIPES THAT CROSSED THE PATH OF THE TRENCH. THE EMPLOYEES WERE ATTEMPTING TO REMOVE THE DIRT UNDER THE PIPES AND CONTINUE THE TRENCH FOR LATER INSTALLATION OF A 15-INCH DIAMETER IRRIGATION PIPE. TWELVE DAYS AFTER THE ACCIDENT, EMPLOYEE #1 DIED FROM COMPLICATIONS RESULTING FROM THE INJURY.
