Investigation Summary
Employee #1 was instructed by his supervisor to uncover a pallet of wood that was on the ground. Next to this were two pallets of wood stacked one atop the other. A 4-wheel drive forklift was being brought in to pick up the pallet to uncover when the top stacked pallet fell on Employee #1. He suffered a broken cheekbone, two broken legs, two broken ankles, a broken left forearm, a broken right shoulder blade, and a cracked rib.
