Investigation Summary
Employee #1 was assigned to unstop an auger conveyor that was discharging waste apple solids from an apple juice processing operation. He was elevated 14 ft to the auger in a basket attached to a forklift, and then climbed out and onto the conveyor auger. His supervisor was working with him and neither of them had locked out the auger before starting work, although the supervisor had told Employee #1 that the conveyor could start up automatically. The supervisor threw a shovel up to Employee #1, who began shoveling when the auger conveyor started. His leg became caught and , crushing his foot and the bones in his lower leg.
