Investigation Summary
Employee #1 was sorting paper for recycling in a work area next to a semi-trailer. A coworker operating a forklift was moving bales of paper to a transport truck, and was backing up slowly when he may have run over a board. A bale fell off the forks, bounced, and struck Employee #1. His shoulder and upper body were pushed against the semi-trailer, and he suffered a bruised shoulder and ribs, and a strained neck.
