Investigation Summary
Employee #1 was on an approximately 12 ft high scissor lift, taping plastic sheeting to the ceiling of the flour room to screen off a machine. As he reached up, he lost his balance and fell backward over the railing and onto the floor. The railing on the lift was lower than the required height and it did not provide adequate employee protection.
