Employee's arm broken when struck by fire extinguisher

Employee #1 was handling and sorting scrap fire extinguishers in a confined area between some pallets and bins. A valve handle assembly broke off a bottle-valve interface, causing the bottle to fly out and ricochet off the walls. Employee #1 was struck by the flying , suffering a broken right lower arm, lacerations, and contusions. He had been instructed not to handle the old fire extinguishers.
