Investigation Summary
Employee #1, a trained, experienced leadsman for Ross and Sons Refrigeration, Inc., was on a 17 ft 8 in. high metal roof, installing and aligning 3 1/2 in. IMC ridged electrical conduit over an alleyway between two buildings. Aligning the two conduits with the help of an apprentice at the other end, Employee #1 kept backing away from a bend in the conduit that was 45 to 50 ft from the lower end, while giving his assistant alignment instructions. He continued backing toward the south edge of the roof (35 ft 6 in. conduit bend to edge), until he fell off onto asphalt. Employee #1 suffered massive head trauma and was transported to Kaweah Delta Hospital, where he died on January 12, 1992. Safety belts and lanyards were not in use on this job, nor were employees using any other form of fall protection.
