Investigation Summary
After installing a new motor on a swamp cooler, Employee #1 was checking the operation of the unit and noticed the belts were loose. The power to the unit was shut off, the belts were tightened, and power was restored. Shortly after power was restored to the unit, Employee #1's right glove became caught in the belt and pulley drive, pulling his hand and index finger around the pulley. His right index fingertip was amputated.
