Employee's leg fractured in fall, later amputated

Employee #1 decided to help his crew finish the siding on a house. He was working from a 12 ft high scaffold, but found that he could not the reach the gable of the roof. Employee #1 then retrieved a 6 ft ladder, took it up to the scaffold platform, and set it against the wall of the house. When he stepped up on the rungs, the legs of the ladder pushed against the scaffold platform and caused it to move away from the wall. Employee #1 fell to the ground and suffered a compound fracture of his left leg. His leg was later surgically amputated above the knee.
