Investigation Summary
At 8:25 a.m. on January 22, 1991, Employee #1 was manually cleaning excess cheese buildup from the molder head pad of an operating cheese molder. The head molder shifted every 10 seconds, and there were six pads on the head. The pad shifted while he was cleaning and Employee #1's right hand and wrist became caught between the pad and the pressure bar. He suffered a broken wrist.
