Investigation Summary
An employee was reading electric meters at a customer-owned facility. The employee had recently been transferred to the area. She was using an electronic meter-reading device and had read three readings from two meters. However, she was not certain that she had taken readings from all the meters at this location, so she decided to check the back side of the electrical cabinet (a transformer enclosure). She unlocked the cabinet, opened it, and looked in. Finding no more meters, she turned to close and relock the cabinet door. As she turned, the employee contacted a 12-kilovolt bus. She received an electric shock and sustained severe burns. She was hospitalized for her injuries.
