Investigation Summary
At 11:45 on November 1, 1991, the forklift platform Employee #1 was working from fell about 8 ft. The platform was secured to the forks as required, but the clips that attached the forks and backguard to the mast carriage came loose. Apparently, the operator had tilted the mast forward to get close to the greenhouse roof and when he went to tilt it back, the platform fell. The employees were pulling plastic sheets off the roofs and a pump located at ground level limited the distance the forklift could be from the greenhouse.
