Investigation Summary
On November 15, 1990, Employee #1, a maintenance helper, and a coworker, a maintenance mechanic, were installing a case palletizer system. The lifting chains on the pallet elevator were not adjusted properly, causing the elevator bed to jam. Employee #1 removed the key from one sprocket and propped the elevator. He was standing on the empty pallet transfer conveyor when the maintenance mechanic started the palletizer. This activated the elevator and started the empty pallet conveyor. Employee #1's lower leg became trapped between the conveyor frame and the moving conveyor drag bar. He suffered a fractured leg and soft tissue injuries. No lockout/blockout procedure was employed.
