Employee injured when crushed in case sealer

On August 7, 1990, Employee #1, a maintenance mechanic for a large frozen food processor, was assigned to clean accumulated glue from the bottom rollers of a case sealer in the fruit processing department. He shut off power to the rollers, but he did not shut off a microswitch circuit that controlled an air-operated ram or the air supply to the ram cylinder. He was reaching in to begin cleaning when the ram actuated, probably due to inadvertent contact with the microswitch. Employee #1's arm and upper body were crushed between the ram and the machine frame, and he suffered extensive soft tissue injuries and damage to his brachial plexus nerve.
