Investigation Summary
At 11:22 a.m. on April 17, 1990, Employee #1, an inmate carpenter's helper, was using a table saw to cut a 1 in. by 7 in. by 14 ft piece of hardwood. He fed the stock from the wrong side of the table and his left index finger was pulled into the saw, amputating the tip. The saw was not guarded.
