Investigation Summary
At 12:45 a.m. on March 20, 1991, Employee #1, a forklift operator for Certified Grocers of California, Ltd., was driving backward toward aisle #30 when he collided with another lift truck that was exiting the same aisle. His left thumb was injured when it became caught between the backs of the two lift trucks.
