EMPLOYEE FRACTURES FINGER IN V-BELT NIP POINT

AT 9:30 AM ON JULY 16, 1990, EMPLOYEE #1 WAS OPERATING A CASE FILLER MACHINE ON HER FIRST DAY ON THE JOB. SHE ATTEMPTED TO REMOVE AN INCOMPLETELY FILLED BOX FROM THE CONVEYOR WITHOUT FIRST STOPPING THE MACHINE AND CONVEYOR, AS INSTRUCTED. SHE INADVERTENTLY PUT THE TIP OF THE 3RD FINGER OF HER LEFT HAND IN THE NIP POINT OF A V-BELT AND PULLEY, WHICH CARRIES THE BOX ALONG. AS A RESULT, SHE HAD A FRACTURE OF THE TIP OF HER FINGER. THE EMPLOYER HAD NOT RECOGNIZED THE POSSIBILITY OR PRESENCE OF THIS HAZARD BUT DEVISED AND INSTALLED A PLASTIC INSERT TO FILL THE NIP POINT AS A RESULT OF THIS INCIDENT.
