Employee's foot injured between pulley and conveyor

At 9:00 a.m. on November 29, 1990, Employee #1, the bottling manager, climbed atop a conveyor located between the palletizer and the shrink wrapper to retrieve a slip sheet that he normally put over the fence guarding the palletizer. The conveyor automatically started, and the heel of his left shoe became caught between an idle pulley and the conveyor at the transition point between two conveyors. He was hospitalized for treatment of injuries to his foot.
