Investigation Summary
Employee #1 was working on the wall of a new concrete structure. He was descending to mix a batch of grout that he was using for finish work on the structure. Employee #1 adjusted an extension ladder so that it was resting properly on the wall. The ladder retracted when Employee #1 stepped on it to descend. Employee #1 fell 9 to 10 feet. The ladder was new and seemed to function properly. The ladder's safety latches catch on the rung when they are pulled up past the rung; this can make it appear that the ladder has been latched properly. It is assumed that this is what caused the accident.
