Employee injured when struck by bell crank arm

At 10:00 a.m. on October 20, 1992, Employee #1 was working as a millwright in the #3 sawmill at Roseburg Forest Products in Anderson, CA. He was called to repair an air valve used to control the ejector cylinders for the storage deck just downstream of the log deck cut-off saw. After repairing the valve, he radioed to the operator to actuate the ejectors. As they were actuated, the rotating bell crank arm came down on top of Employee #1's hard hat, driving him to the ground and crushing a disc in his spine at the neck. A subsequent investigation showed that the employee was well trained and experienced and that a lockout program was in place and had been followed. No violations of regulations were found.
