Employee's hand injured when caught between pipe and beam

At approximately 6:05 a.m. on November 12, 1990, Employee #1 was standing on an operating box link-type chain conveyor, trying to clear a jam of scrap wood that had piled up on the sides and bridged the conveyor. He had braced one end of a 1 1/4 in. by 5 ft long piece of pipe against a link of the moving chain conveyor below the jam, and held the upper end of the pipe in his right hand. As the conveyor pulled the pipe through the jam, the end of the pipe he was holding ran up against an overhead I-beam. Employee #1's hand sustained severe lacerations and fractures.
