Investigation Summary
At approximately 12:00 noon on October 31, 1990, Employee #1 was pressing an end fitting onto a piece of aluminum electrical wire. A filler compound used to coat the wire to prevent oxidation squirted out of a relief opening in the end fitting, striking Employee #1's eyes. He was not wearing eye protection.
