Employee's fingertip amputated by belt and pulley

On October 16, 1991, Employee #1, a truck driver for Doyle and Joan Haskins Farms, was hauling potatoes from a field to a storage cellar with a bulk potato truck. The truck has a conveyor at the bottom of the truck bed to offload the potatoes. The conveyor is driven by a V-belt and pulley drive that is electrically controlled. Employee #1 plugged the drive motor in and it began to operate. The motor turned, but the V-belt did not. This meant the conveyor was not moving. Employee #1 took a stick and pushed the V-belt. He was trying to create enough tension on the belt and pulley so the belt would move the conveyor. The belt moved suddenly, pulling Employee #1's finger into the belt and pulley. The tip of his finger was amputated.
